I, , hereby engage located at (hereafter, "Salon") for the cosmetic treatment of my nails, skin, hair and/or body and agree to abide by all Salon policies posted in the store or on its website. In addition, I agree to the terms of this waiver of liability, assumption of risk and indemnity agreement.
I hereby certify the following:
That I have not traveled overseas in the last 14 days.
That I have not come in close contact with anyone who has traveled overseas in the last 14 days.
That I have not come in close contact with or cared for anyone who has been diagnosed with COVID-19 in the last 14 days.
That I have not experienced any cold or flu-like symptoms in the last 14 days, including fever, cough, sore throat, respiratory illness or difficulty breathing.
I also understand and agree to the following:
That the visit to Salon requires a verbal screening, the wearing of cloth face coverings during the entirety of the visit, and the use of hand sanitizers.
That the visit to Salon may result in exposure to communicable diseases, such as COVID-19.
That the visit to Salon and the application of products and services offered by Salon and its employees may result in illness, injury, allergic reactions, aggravation of pre-existing medical conditions, permanent disability or death.
That it is solely my responsibility to seek out medical attention to treat adverse medical conditions.
That protective gear, physical barriers, disinfectants, social distancing protocols, first aid, advice and literature provided by Salon and its employees have no implied warranties.
I hereby assume all risks inherent to engaging Salon and further release, waive, indemnify, discharge and covenant not to pursue litigation against Salon, its employees, its suppliers, its owners, and its landlord from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any physical injury or death or emotional distress or theft or damage to personal property or lost wages that I may sustain by engaging Salon.
